What are My Rights and Responsibilities as a Patient?
As a patient of The Hand and Upper Body Rehabilitation Center, you can expect certain information and treatment from our staff. These are your “rights” as a patient. You are a partner in your own healthcare. Therefore, you have a right to:
- Be informed about your health status or the status of your injury, and be involved in your care and treatment.
- Be given instructions about your continuing health needs, which may include a home program to be followed to achieve your rehabilitation goals.
- Receive expert and quality care from trained therapists and staff.
- Consent or decline to be in any research studies that may be proposed to you.
- Seek advice from another healthcare professional if you choose.
- Know or be able to ask about any business relationship between our facility, payers and other providers that might affect your treatment or care.
- Expect confidentiality of your records, except as allowed by law or insurance contracts.
- Ask to see the information in your record and request a copy.
- Be free from discrimination on the basis of any legally protected characteristic.
For you to receive the best possible care, Hand and Upper Body Rehabilitation is counting on you to work with us. As a partner in your care you are responsible for the following:
- Taking an active part in your care by following the treatment plan advised by your therapist and/or physician.
- Reporting any changes in your condition to your therapist.
- Wearing any orthotics or splints as prescribed by your therapist and/or physician.
- Understanding the plans made for your treatment and asking any questions should you not understand what is being required of you.
- Providing us with the information needed for your insurance.
- Keeping all appointments made for you, and informing our office in advance of cancellations.
As your therapy provider we pledge to give you the best possible care. We welcome your comments about the care you receive. If you have any comments, questions or concerns, please feel free to speak with any Hand and Upper Body staff member.
